ROSTER ELIGIBILITY

Please print first & last names as shown on  the player's FHF or USFHA  membership card.
All players must be members of either the USFHA or FHF.
Date of birth for all players in the "Under 16 & 13 Divisions" must be listed. This will be used for player
verification & eligibility purposes.
Ten players registration is included with a team's entry. Two additional players can be added at $40 or $10 per
player depending on division entered. Maximum number of players in a team is twelve.
A minimum of 6 players must be listed for all divisions in order to enter a team. Names entered may be
changed or deleted by submitting a new form at least 30 minutes before a team's first match.
California Sixes or Field Hockey Federation does not provide medical insurance for participants. All players
must have medical insurance which is valid in the United States of America.
All players under the age of 18 must complete the Emergency Information & Consent to Treat Form.
